Step 1
Navigate to All Forms
In the left navigation, under the CRM area, click “All Forms.” This page shows all of your existing forms. By default, you’ll see a “General Leads Template” that comes pre-loaded in your account. You can edit this template to fit your needs, or create a new form from scratch.

Create a New Form
Click “Add New Form” in the top-right corner. The form builder will open, showing the form fields in the center of the page.
On the right side, you’ll see two types of fields you can add:
Predefined fields — standard fields like Name, Email, Phone, and Message that are ready to use.
Custom fields — scroll to the bottom of the field list to create your own fields for any data your form needs to capture.
Drag fields into your form to build it out. The goal is to match the fields on your form to the same fields that exist on your website’s contact or lead form.
Step 3
Save and Assign to a Company
Once your form fields are set up, click “Save.” When you save, you’ll be prompted to assign the form to a company. Select the company (or companies) that should use this form.
Important: Always assign companies to your forms. If a form isn’t assigned to a company, the system won’t know where to route incoming submissions, and they may not appear correctly in your CRM. |

Understand the Form Editor Tabs
When you click “Edit” on an existing form, you’ll see four tabs at the top of the page:
Tab | What It Does |
Form Builder | Add, remove, or rearrange the fields on your form |
Companies | View and manage which companies are assigned to this form |
Field Mapping | Map your website’s form fields to LeadSnap’s internal fields |
Form Mapping | Assign which form each company should use |

Set Up Field Mapping
Click the “Field Mapping” tab. Select a company from the dropdown at the top. As long as that company is assigned to this form, all of the form fields will appear with two columns:
Website fields — the field names as they appear on your website’s form.
LeadSnap fields — the internal fields in LeadSnap where the data should be stored.
Map each website field to its corresponding LeadSnap field so the system knows exactly where each piece of incoming data should go.
Important: If fields aren’t mapped correctly, form submission data may end up in the wrong place or not appear at all. Always verify your mapping after setting it up by submitting a test form. |
Step 6
Set Up Form Mapping
Click the “Form Mapping” tab. This page shows a list of all your companies. On the right side of each company, you’ll see a dropdown where you can select which form that company should use.
Use this page to quickly assign or reassign forms across your entire company list in one place.
Tip: Form Mapping is especially useful when you manage many companies. Instead of editing each form individually to assign companies, you can see and manage all assignments from this single page. |
